Supplemental Medical Form for Summer Camp
Heritage Scout Reservation is once again requiring all scouts to arrive at summer camp with the Laurel Highlands Council Supplemental Medical Form, which must be completed and signed by a parent.
Please complete the attached form and bring it with you to the swim test on June 5 or the troop picnic on June 12.
We realize that this form duplicates information you have already completed on the BSA Medical Forms A&B, but it is a requirement of the camp. Thanks for your attention to this and your understanding!
