All family members attending the Father-Son camp this weekend need to turn in a BSA Medical Form Part A&B with a photocopy of a current medical insurance card. These pages of the medical form DO NOT require a physician’s signature.
Please complete the forms and the insurance card photocopy before camp, so our leaders can collect them when you arrive. Please plan to retrieve your forms after camp, especially if you intend to travel to Mammoth Cave in June. All unclaimed BSA_Annual_Health_and_Medical_Forms_A&B_only.pdf forms will be shredded.
