If your Scout attended summer camp, you brought a copy of his medical form and gave it to the camp staff at the time of check-in, while a copy remained with the Troop in the Troop medical file box. We have those extra copies available for you to take back and keep on file, if you wish.
The spare copies will be located in a file box at the check-in table for the Tuesday troop meeting at St. John Neumann. The medical forms are in envelopes, alphabetical by last name. We will keep the forms available for you for the next three weeks. After the Troop Court of Honor on October 6, all remaining spare copies of medical forms will be shredded.
